Insurance Information
As of January 1, 2011, we will no longer accept insurance. Most PPO and indemnity plans will reimburse some of the cost. We will keep our fees in line with the typical reimbursement of the better paying plans such as Aetna (this is still lower than a typical office charge). If you have questions about your plan, it is best to address these with the customer service representative whose number is printed on the back of your card.
We will provide you with an invoice at the end of your visit that you can submit to your insurance company either via fax or mail.
